On 29 January 2025, a US Army Sikorsky UH60L helicopter conducting pilot training was in collision with a scheduled service Mitsubishi CRJ700 on final approach to runway 33 at Washington National in night VMC. There were no survivors. The Investigation found that the helicopter route’s designation close to a runway approach path and failure of the helicopter pilots to maintain an adequate lookout or remain within the maximum designated route height as well as longstanding systemic deficiencies in respect of air traffic control and army operational safety management were contributory and noted limitations in collision alerting systems on both aircraft.
